Sparked Chronic Condition Management Clinical Focus Group
Welcome to the Sparked AU Patient Summary Clinical Focus Group
The Sparked Australian Patient Summary Clinical Focus Group (AU PS CFG) is a sub-group of the Sparked Clinical Design Group (CDG) which will provide support to enable to the development of Patient Summary within the Sparked program.
The AU PS CFG will be a time limited committee until 30 June 2025, subject to the Sparked Program requirements.

Terms of Reference
Objectives and Responsibilities of the AU PS CFG
The role of the Sparked CCM CFG is to provide targeted clinical support to enable the development of Chronic Condition Management priorities within the Sparked FHIR Accelerator, this includes but is not limited to:
- Create example clinical workflows and/or scenarios related to management of a chronic condition to help development of the AUCDI and FHIR Implementation Guides, and support the progress of the Sparked CDG and the wider Sparked FHIR Accelerator program
- Creation of journey(s) highlighting the complexities of chronic condition management.
- Identifying considerations regarding chronic condition management template(s) which supports team care/shared care.
- Provide clinical input and insight to relevant FHIR IGs and/or Technical Design Group(s) as required
- Support AUCDI development as required by the Sparked CDG
- Assist in developing test data or materials to support clinical education and understanding of CCM (if required)
Out of scope
- Broader discussions regarding policy reforms, technical implementation, funding arrangements or requirements, not relevant to the Sparked FHIR Accelerator.
- Determining or defining specific clinical care pathways, guidelines, or treatment recommendations.
- Materials developed in this forum are indicative clinical journeys or workflows, and do not specify a required or recommended clinical process or pathway.
- Not comprehensive or inclusive of all possible scenarios
Meetings
Meetings will be held as required to support the Sparked FHIR Accelerator program of work.
It is expected that meeting may be held on a fortnightly and/or monthly basis during the initial development stages, and transition to a monthly or bi-monthly basis as required during the later development stages.
Focus group meetings may be stood up on an ad-hoc basis to progress specific prieces or work or meet program timeframes or deadlines.
The CCM CFG meetings are time limited to align with Sparked FHIR Accelerator program deliverables and requirements.
The CCM CFG meeting shall chaired by a member of the Sparked project team and/or by a member of the CCM CFG.
Goverance structure
The CCM CFG is a sub-group of the Sparked Clinical Design Group, that will not be a decision-making body.
All minutes, artefacts and outputs from the CCM CFG will be made available to the members of the Sparked CDG.
Additionally, relevant outputs shall be shared directly to the relevant Sparked Technical Design Group/s (TDGs) and Sparked FHIR IG Project Team(s) as requested or required to support development.
Membership
The CCM CFG will be made up of a approximately 20 independent clinical experts or consumer representative members. Members have been selected to span the clinical perspectives from a variety of different clinical professions (e.g. doctors, nurses, allied health, aboriginal health workers, etc) and care settings (primary, acute, aged care, community, etc)
Membership of the CCM CFG will comprise of selected Sparked CDG members that have submitted an Expression of Interest (EOI) via the Sparked Chronic Condition Management Clinical Focus Group EOI MS Form which was made available to the Sparked Clinical Design Group (Appendix A).
All discussions in the CCM CFG shall be done so without prejudice, in a collaborative and inclusive manner to enable participants to genuinely share insights, expertise and advice.
Observers
Observers may be invited to attend to gather insights on group dynamics, participant engagement, and overall discussions.
Observers should refrain from participating in or influencing the discussions.
Conflict of interest
Members are expected to declare any conflicts of interest, where they arise.
All minutes, artefacts and outputs from the group will be made available to the broader Sparked community via the Sparked CDG, relevant Sparked FHIR IG Project Team(s) and/or Sparked Technical Design Group(s) as required to enable and support the Sparked FHIR Accelerator program of work.
Withdrawal from the CCM CFG
Members may withdraw from the CCM CFG at any time. It is important to note that data collected up to the point of withdrawal remains a valuable part of the project, and personal information will be managed with care, respecting privacy and confidentiality.
