Clinical Design Group & Community Co-Design Workshops 29 & 30 July 2025 (In-person Sydney)


Join the community of clinicians, software developers, vendors, jurisdictions and health informaticians to discuss and contribute your expertise as we begin the development of AUCDI R3. The focus of Day 1 workshops will be on Chronic Condition Management and health assessments, including scales, scores and observations. As well as looking at prioritisation of the AUCDI Backlog. Workshops on Day 2 will be a deep dive into the clinical/technical processes and workflow for Chronic Condition Management, build on the consumer journeys developed by the Sparked CCM Clinical Focus Group.

Agenda: Day 2

Overview: Day 2 is focused on Chronic Condition Management (CCM). Building on our work from the Sparked Chronic Condition Management Clinical Focus Group, these workshops will look at building a roadmap for CCM with a specific focus on user experience and design requirements, and how the work on AU Patient Summary, eRequest and FHIR enabled Smart Forms can support interoperable data flows and enhance the clinician and consumer experience. The outcomes of the day will identify minimum requirements for a FHIR enabled GP Management Plan and a roadmap for future development of data models, specifications and workflows as a foundation for enhanced CCM.
